Waiver & Release
Refund & Credit Policy
Cancellations made more than sixty (60) days prior to the start of the camp are eligible for a partial refund equal to fifty percent (50%) of the total registration fee paid.
Cancellations made within sixty (60) days of the camp start date are non-refundable.
All refund determinations are based on the camp start date. No refunds will be issued for late arrivals, early departures, no-shows, injuries, illness, or dismissal from the program for any reason.
Parental / Legal Guardian Certification
I/We, the undersigned, certify that I/We am/are the parent(s) or legal guardian(s) of the registered participant and have the legal authority to execute this agreement on their behalf. I/We further certify that all information provided during registration is true, complete, and accurate to the best of my/our knowledge.
Consent for Medical Treatment
I/We hereby authorize White Eagles Soccer Academy, its staff, coaches, and representatives to seek, authorize, and obtain emergency medical treatment for my/our child in the event of injury, illness, or medical emergency during participation in any Academy activity.
I/We understand and agree that all medical expenses incurred are my/our sole responsibility, except to the extent covered by any applicable excess medical insurance maintained by the Academy, if any. This authorization includes, but is not limited to, transportation by ambulance, hospital admission, and treatment by licensed medical professionals.
Assumption of Risk
I/We acknowledge and understand that participation in soccer and related athletic activities involves inherent risks, including but not limited to:

Physical contact with other participants

Falls, collisions, and overexertion

Weather conditions
-
Playing surface and facility conditions

Equipment-related injuries
I/We certify that the participant is physically and mentally capable of participating and has no medical conditions that would prevent safe participation unless disclosed in writing to the Academy prior to participation.
Waiver & Release of Liability
In consideration of acceptance into any White Eagles Soccer Academy program, I/We voluntarily assume all risks associated with participation and hereby waive, release, and discharge White Eagles Soccer Academy, its directors, officers, employees, coaches, volunteers, agents, contractors, affiliates, successors, and assigns from any and all claims, demands, actions, causes of action, liabilities, damages, or expenses of any kind arising out of or related to participation, including those resulting from ordinary negligence, to the fullest extent permitted by law.
This release applies to all injuries, damages, or losses to person or property, whether occurring on or off the field, during practices, games, camps, clinics, travel, or related activities.
Early Withdrawal
I/We acknowledge and agree that no refunds will be issued in the event of early withdrawal, dismissal, or removal from any program for any reason, including injury, illness, scheduling conflicts, or personal circumstances.
